What Are
Health DIsparities?

Health disparities are unfair and avoidable differences in health status among people groups. Disparities in health and wellness exist within neighborhoods, cities, and countries for many reasons, including living environment, access to quality health care and education, finances, and social support. These reasons are also called "Social Determinants of Health", described by the World Health Organization as the conditions in which people are born, live, and work, all of which are influenced by how resources, money, and power are distributed locally and globally.

In New York City, the U.S. and around the world, racial and ethnic minority groups and people with low-income are negatively impacted by health disparities, leading to higher rates of chronic disease and health complications and shorter lifespans. Lowering the risk of chronic disease and improving health for all requires more than medical care. To solve health disparities and promote health equity, which is the fair opportunity everyone should have to be as healthy as possible, it is important to address the Social Determinants of Health and ensure that all people have access to the resources they need to thrive.

What are chronic diseases?

Chronic diseases are conditions that have lasting health consequences and usually require long-term treatment. These conditions can be managed or controlled but are usually not curable.

 

There are many chronic diseases including cancer, heart disease, stroke, diabetes, and arthritis, and more. For a detailed list see Chronic Diseases and Conditions.  Cancer, and heart disease are among the leading causes of death in the United States and across the globe. The COMMUNITY Center currently focuses on cardiovascular disease, cancer, and the issues that lead to increased risk of developing a chronic disease in the New York City region.
